Membership in our In-House Loyalty Program is available to all patients. After payment of an annual fee, members will receive the following:
Periodontal Cleanings:
Exam, Digital X-ray (if necessary), Dental Cleaning, Oral Cancer Screening, Oral Hygiene Instructions, TMJ Screening, Periodontal Evaluation
Unlimited Emergency Visits:
• Problem Focused Exam
• Single X-ray (If necessary)
20% Discount Off Our Customary Fee:
A treatment plan will be provided in writing for all recommended procedures displaying the regular fee and the discounted rate.

PRICES:
Adult: $350/year • Couple $600/year • Child: $250/year
Coverage is in effect for one year from sign up.
Coverage begins when membership is paid in full.
Members will be notified one month prior to membership renewal.
